New From The Innovation Lab: AI for Fewer Denials
By Blakely Roth | October 21, 2026
Two new releases from the Clearwave Eligibility Agent that support denial prevention.
Claim rejections and denials are constantly a top challenge for any healthcare organization, and leaders are looking for new solutions to speed up cash flow. Of the providers that are using AI to support claims process, 70% say this approach reduces denials and resubmissions.
Clearwave has released two new features built to reduce claim denials by fixing insurance data earlier in the patient journey. AI Insurance Card Capture reads a patient’s card at booking and pre-fills their coverage details and Eligibility Flag Assist automatically clears the routine flags that administrative teams are constantly resolving. See how these releases will help healthcare organizations process cleaner claims from the get-go.
Why do Rejections and Denials Start Long Before Billing?
Most rejected claims trace back to something that went wrong days or weeks before anyone opened the billing queue, usually a wrong digit in a member ID or an eligibility flag that got dismissed in a hurry. By the time the claim comes back to billing rejected or denied, the visit has already happened and the work to fix the claim issue costs far more than getting it right the first time would have.
The hard part for most practices is that the fix has never lives in one place. Better data entry helps at the front desk, better automation helps in the middle and better reporting helps leadership enhance performance going forward, but each one is a separate project. Clearwave covers all these areas with one agentic workforce.
AI Insurance Card Capture: Clean Data from the First Touchpoint
Insurance data has traditionally been the least reliable piece of the patient record because it starts as a patient typing a long string of numbers and letters into a form on their phone. AI Insurance Card Capture, from the Clearwave Scheduling Agent, replaces that step with a seamless process that patients are already accustomed to.
Patients upload a picture of their insurance card while they book, and the Scheduling Agent reads the member ID, payer and plan details and pre-fills them into the account. The patient confirms in seconds, with no typing and no re-entry. By the time staff pick up pre-appointment work, the account already holds a verified card image alongside pre-filled coverage information.
What practices get
- More completed bookings. One quick photo replaces a form full of fields, so the booking flow stays short and more patients finish it.
- Hours back for staff. Nobody re-types card details by hand, because every booking arrives with pre-filled, patient-confirmed data sitting in the PM System.
- Fewer denials. Getting accurate coverage details at the very start protects revenue and cuts the rework that follows a rejection.

Eligibility Flag Assist: Let the Routine Flags Clear Themselves
Eligibility Flag Assist is the newest innovation from Clearwave’s Eligibility Agent. It absorbs the routine work staff dread by automatically clearing common flags for known insurance verification patterns, like a routine Service Type or a common dual-coverage combination. Clients define their rules once, and Clearwave will automatically resolve those flags as they arise, reducing another task on the to-do list.
Two common flags, automatically taken care of
- Contact Other Entity Flag. Clients can map a Visit or Appointment Service to the Service Type Codes they expect, and the flag auto-closes when it finds a match.
- Other Payer-on-File Flag. Clients can define the payer pairs they see constantly, such as Medicare primary with Medicaid secondary, and the flag clears when a patient’s coverage matches their preferred rule.
What administrative teams are left with is a shorter queue made up of the flags that actually carry denial risk, which means the exceptions get the attention they deserve. Because a rule applies the same way at every site, it also removes the location-to-location variation that quietly produces rejections at one office but not another.

How These Enhancements Work Together to Drop Denials
Separately, each release is useful, but together, these innovations support the same problem at two key moments. AI Insurance Card Capture ensure patient data accurate when it’s first entered into the system, Eligibility Flag Assist reduces the noise so staff can focus their attention on the complex issues that require their full attention.
This progression is what agentic automation looks like in practice. The Scheduling Agent and Eligibility Agent take on the repetitive work alongside staff, the Workflow Agent watches each hand-off and escalates what’s needed to human agents. Clearwave has run 2B+ eligibility checks to date, and every one of those checks is only worth as much as the follow-up behind it.
